How To Request Pricing for Nursing CNA Kits

1. Select the Standard Base Kit, Professional Base Kit, or the Total Custom Kit option.
2. Click on the Quote Form link for the kit option you have chosen. Download the MSWord document (.doc format)
3. Fill out the form, selecting the additional items which will customize your kit, and determine quantities for each item. If you want items not found on this site, make a note to ask us - we have many items available!
4. Determine the number of kits you need, and when you will need delivery (our lead time is 4-6 weeks from order date). Please note that the minimum order quantity is 20 kits.
5. Email or FAX your completed form to us. Click here to go to the Contacts page to find your sales representative's contact information.
6. You will receive a formal quote via email. Please note that freight is not included in your quote. If you would like a freight quote, please note on your Quote Form.
